DO I NEED A DOCTOR’S REFERRAL?
Colorado is a direct access state, so most insurance companies do not require a prescription, however MEDICARE currently does in the state of Washington.
WHAT IS YOUR NEW PATIENT PROCEDURE?
In order to maximize our time together during your initial visit I strongly encourage you to fill out the forms on the "New Patient Registration” link below. If you're unable to complete forms online prior to your visit please arrive to your initial visit 15 minutes early in order to complete them then.
WHAT TYPES OF HEALTH INSURANCE DO YOU ACCEPT?
EBPT is an in-network provider with Medicare. All other insurance companies are out of network; however per patient request EBPT can create an itemized super bill and receipt for the services provided to you and will submit to your insurance for reimbursement. In this scenario the money you pay for physical therapy will be counted towards your deductible.
WHAT IS YOUR CANCELATION POLICY?
If you do not notify EBPT of a cancelation within 12 hours of your appointment you will be charged $60. If you do not show up for a scheduled appointment without any notice you will be charged the full amount for your missed appointment.

EBPT is able to offer a competitive cash pay rate discount because the business does not contract with private insurances.

Getting to EBPT
EBPT is located in the Mittenwalder Platz building. There is plenty of parking available out front. We are conveniently located on the first floor and handicap accessible. At EBPT we recognize the importance of offering therapy in a private and safe space to encompass a therapeutic environment. Please do not disturb the session prior to yours.
